You are financially responsible for all services rendered, regardless of insurance coverage. Payment terms include:

Co-payments are due at the time of service

Deductibles and co-insurance are due upon receipt of insurance processing

Self-pay patients are expected to pay in full at time of service

We accept cash, checks, credit cards, and approved payment plans

Insurance

While we will submit claims to your insurance company as a courtesy, you are ultimately responsible for payment. It is your responsibility to verify your insurance coverage and benefits prior to receiving services.

Past Due Accounts

Accounts not paid within 90 days may be subject to late fees and may be sent to a collection agency. You will be responsible for all collection costs, including attorney fees.
